Create a Learning Organization

One thing that I've seen work really well is for leaders to simply state that their organization is a "learning organization" and to continue to reinforce that message through their actions. Semiconductor is notorious for "quality" meetings which on the surface come across as a "way to improve quality" but end up feeling like an inquisition. A great way to get people to stop trying is to set up punitive meetings. Try to shift the focus from blaming to learning so that people recognize they have to be accountable but that they have freedom to try something new and innovative.
